PACE PEER Program Enrollment Form

Commitment and Guidelines to Participate in the PACE PEER Program

  1. Active Participation: Attendees are committed to actively engaging in the 90-minute monthly facilitated Zoom sessions. In the event of unavoidable absence, attendees are encouraged to designate a substitute to participate on their behalf at the table.
  2. Engagement in Roundtable Discussions: Attendees agree to engage constructively in roundtable discussions, contributing ideas, sharing experiences, and offering support to fellow participants.
  3. Commitment to Learning: Attendees commit to learning and implementing new strategies and best practices discussed during the sessions to enhance their enrollment efforts.
  4. Accountability and Follow-Up: Attendees agree to embrace accountability by following up on action items discussed during the sessions and sharing progress with the group.
  5. Respect and Professionalism: Attendees commit to maintaining a respectful and professional demeanor during all interactions within the program, fostering a positive and inclusive environment.
  6. Confidentiality: In the group, information will be shared. However, if someone requests that certain information not be shared, attendees agree to respect that request and maintain confidentiality.
  7. Continuous Improvement: Attendees commit to seeking continuous improvement in their enrollment strategies and leadership skills, leveraging the resources and support provided by the program.
  8. Payment Commitment: Attendees commit to fulfilling their financial obligations based on the selected number of PEER Members and requested additional strategy sessions. Projects are structured according to their scope and are remunerated through project-based installments.
  9. Timeliness: Attendees commit to attending sessions punctually and respecting the allocated time for discussions to maximize productivity and effectiveness.
  10. Feedback and Evaluation: Attendees agree to provide constructive feedback and participate in program evaluations to help improve the quality and relevance of future sessions.

  

By enrolling in the PEER Program, I acknowledge and agree to the terms outlined above. 

Investment

Upon signing this agreement and securing payment, MHT will schedule an initial strategy session to establish your benchmarks and goals for the program.  


First Member: $4,800 per year 

Second Member: $3,600 per year 

Third Member: $3,600 per year 

Fourth Member: $2,400 per year

Payment Options

10% discount for payment in full. 

 

Quarterly payments available:

  • Payment 1: Due immediately
  • Payment 2: Prior to 2nd strategy session
  • Payment 3: Prior to 3rd strategy session
  • Payment 4: Prior to 4th strategy session
